I miss the word "urodzony" ahead of >w< Morkowie.

As far as I know, in a term like: "Malzonkow Rachuiow z Morkowie" usually 
Morkowie would state the maiden name of the mother.
Thus I would translate:
"Syn Woyciecha i Anny Elzbiety Malzonkow Rachuiow z Morkowie"
as:
"son of Adalbert Rachui and Anna Elisabeth nee Morkow"
