The place could be what is listed in Jerry Frank's index as Muntowka, at C-2 on his map.  I think this must be near Vladimir Volynsk.

We have been able to translate the following - 

July 6th birth date, Baptism is July 7th 1885 - the name is Wilhelm Bohn. 

Parents are August Bohn and Regine Weiss. Godparents Andreas Wagner and Julianna Weiss. 

Just need the place.
